Influence of various factors on the effectiveness of sinus lifting and the role of augmented depot morphology: a retrospective clinical study

Abstract

Abstract

Background: Variations in shape that arise not as the surgeon's intentional design, but as a result of the clinical situation, correlate with the long-term stability of the remodeled bone.

AIM: To assess the influence of the morphology of the augmented subantral site and the applied surgical modalities on the long-term outcomes of sinus lifting and the survival of dental implants.

Methods: A retrospective study was conducted on 193 patients who underwent consecutive surgeries at the medical facilities of Chechen State University named after A. A. Kadyrov. Depending on the configuration of the augmented site, five modalities were distinguished: dome-shaped elevation, recess filling (folds), uniform elevation of the basal membrane, tuber-like configuration (closed sinus lifting), and island-like deposits. CBCT control was performed at 6 months, 1 year, 3 years, and ≥5 years. Implant survival and augmented bone stability were evaluated. The analysis was descriptive in nature.

Results: Over a period of ≥5 years, the highest frequency of successful outcomes was observed with recess filling (100%) and uniform elevation (around 99%), slightly lower with dome-shaped elevation (around 83%) and mound-shaped configuration (around 62%), and the lowest with island deposits (around 33%). Uniform and even distribution of the graft and targeted correction of recesses were associated with minimal resorption, absence of significant inflammatory complications, and implant survival of 98–100% at the five-year mark.

Conclusion: The morphology of the augmented site is a key determinant of bone remodeling and implant stability after sinus lifting. The preferred methods should be considered uniform elevation and recess filling; island deposits show low predictability and are not recommended in cases of severe maxillary atrophy.
