The relevance of the study is due to the need for long-term immunosuppressive therapy in transplant recipients, which significantly affects the condition of the oral tissues.</p> <p>Special attention is paid to early diagnosis, analysis of clinical manifestations, and characterization of the course of pathological processes under immunosuppression.</p> <p>A clinical case of a 64-year-old patient with stage 5 chronic kidney disease (hypertensive nephropathy, regular hemodialysis since 2021), kidney allograft, type 2 diabetes mellitus, multinodular goiter, and cholelithiasis is presented. The patient was hospitalized in the kidney transplant department for routine evaluation and treatment adjustment (successful kidney transplantation was performed in 2023). Examination revealed an increase in serum creatinine to 290 µmol/L. Hemodialysis was initiated and an arteriovenous fistula was created.</p> <p>Oral examination revealed a dull, pale pink mucosa of the cheeks, palate, tongue, and alveolar ridges with signs of insufficient moisture. Teeth 2.5, 2.6, and 1.7 were retained roots with painless percussion; the simplified oral hygiene index was 1.6 (satisfactory hygiene); the DMFT index (decayed, missing, and filled permanent teeth) was 17 (high). Pronounced oral changes were noted, including xerostomia and multiple areas of tongue desquamation accompanied by pain, hyperemia, and fibrinous deposits. These changes developed six months after kidney allotransplantation and are presumably associated with the state of immunosuppression.</p> <p>Based on the clinical observation, we illustrate the diagnostic difficulties caused by the polymorphic clinical picture and similarity of symptoms to other oral mucosal diseases.</p> <p>We emphasize the need for an interdisciplinary approach to the management of this patient group with mandatory involvement of a dentist.</p> <p>Dental adverse effects in patients after kidney transplantation increase the risk of infectious complications and require regular monitoring, meticulous oral hygiene, and comprehensive follow-up through interdisciplinary collaboration.</p></abstract><trans-abstract xml:lang="ru"><p>В настоящей статье рассматриваются фармакоиндуцированные поражения слизистой оболочки рта у пациентов после трансплантации почки на примере клинического наблюдения. Актуальность исследования обусловлена необходимостью длительного применения иммуносупрессивной терапии у реципиентов трансплантата, оказывающей существенное влияние на состояние органов и тканей рта.</p> <p>Особое внимание уделено вопросам ранней диагностики, анализу клинических проявлений и характеристике особенностей течения патологических процессов в условиях иммуносупрессии.</p> <p>Представлен клинический случай пациента 64 лет с хронической болезнью почек 5-й стадии (гипертоническая нефропатия, программный гемодиализ с 2021 года), аллотрансплантатом почки, сахарным диабетом 2-го типа, многоузловым зобом и желчнокаменной болезнью, госпитализированного в отделение пересадки почки для планового обследования и коррекции терапии (в 2023 году проведена успешная трансплантация почки). При обследовании у пациента диагностировано повышение концентрации креатинина до 290 мкмоль/л. Начат гемодиализ, сформирована артериовенозная фистула.</p> <p>В ходе осмотра ротовой полости слизистая оболочка щёк, нёба, языка, альвеолярных отростков — матовая, бледно-розовая, с признаками недостаточной увлажнённости. Зубы 2.5, 2.6, 1.7 — корни, безболезненная перкуссия; упрощённый индекс гигиены полости рта — 1,6 балла (удовлетворительная гигиена); индекс интенсивности кариеса зубов (сумма кариозных, пломбированных, удалённых постоянных зубов) — 17 (высокий). Выявлены выраженные изменения слизистой оболочки рта, включающие ксеростомию, а также множественные участки десквамации слизистой оболочки языка, сопровождающейся болевым синдромом, гиперемией и фибринозным налётом. Указанные изменения развились через 6 мес после аллотрансплантации почки и, вероятно, ассоциированы с состоянием иммунодепрессии.</p> <p>На основании представленного клинического наблюдения продемонстрированы диагностические трудности, обусловленные полиморфизмом клинической картины и сходством симптоматики с другими заболеваниями слизистой оболочки полости рта.</p> <p>Подчёркивается необходимость междисциплинарного подхода при составлении плана курации данной группы пациентов с обязательным участием врача-стоматолога.</p> <p>Стоматологические проявления нежелательных эффектов у пациентов после трансплантации почки способствуют повышению риска инфекционных осложнений и требуют регулярного мониторинга, тщательного соблюдения гигиены рта, а также комплексного динамического наблюдения в рамках междисциплинарного взаимодействия специалистов.</p></trans-abstract><kwd-group xml:lang="en"><kwd>kidneys</kwd><kwd>oral cavity</kwd><kwd>immunosuppressive therapy</kwd><kwd>case report</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>почки</kwd><kwd>ротовая полость</kwd><kwd>иммуносупрессивная терапия</kwd><kwd>клинический случай</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Albagieh H, Alosimi A, Aldhuhayan A, et al.
