Of 87 screened patients, 70 individuals aged 50–75 years (mean age 64.7 ± 6.6 years; 35 men, 35 women) with complete edentulism (Schroeder/Keller class I–II atrophy) were allocated using sequentially numbered, opaque, sealed envelopes (SNOSE) into two groups (n = 35 each): group 1 — CAD/CAM complete removable dentures; group 2 — conventional acrylic dentures. Follow-up was performed at baseline (T0), at 14 days (T1), at 1 month (T2), and at 3 months (T3). The primary endpoint was the total OHIP-14 (Oral Health Impact Profile) score; secondary endpoints included visual analogue scale (VAS) satisfaction across 5 domains, adaptation time, and number of adjustments. The primary analysis was performed on a per-protocol basis (n = 60); an additional intention-to-treat (ITT) analysis (n = 70) was conducted using multiple imputation (MICE, m = 20) in Python 3.11.</p> <p>RESULTS: No significant differences were observed between groups at baseline (p = 0.34). At T3, the OHIP-14 score was 15.7 ± 3.6 in the CAD/CAM group versus 22.7 ± 4.7 in the conventional group (Δ = 6.99; 95% confidence interval (4.84 to 9.14); p &lt; 0.0001; d = 1.64), exceeding the minimal clinically important difference (minimal clinically important difference = 5). The mean VAS score was 81.4 ± 5.8 mm in the CAD/CAM group versus 62.9 ± 7.1 mm in the control group (Δ = 18.5 mm; p &lt; 0.0001). Adaptation time decreased from 17.2 ± 4.8 to 9.0 ± 3.3 days (p &lt; 0.0001); the number of adjustments decreased from 2.88 ± 1.78 to 1.56 ± 1.48 (–45.8%; p = 0.003). "Excellent" ratings were given by 92.6% of patients in the CAD/CAM group versus 24.2% in the control group (p &lt; 0.0001). The ITT analysis confirmed the robustness of the findings (Δ = 7.39; p = 0.003; d = 1.03).</p> <p>CONCLUSION: CAD/CAM-fabricated complete removable dentures provide statistically and clinically significant advantages over conventional acrylic dentures in terms of quality of life (OHIP-14), patient satisfaction (VAS), adaptation time, and number of adjustments. These findings support the priority implementation of the CAD/CAM protocol in clinical practice and provide an evidence base for its integration into the teledentistry system for residents of remote, rural, and hard-to-reach areas of Russia.</p>
<trans-abstract xml:lang="ru"><p>Обоснование. Полученные результаты обосновывают приоритетное внедрение CAD/CAM-протокола полного съёмного протезирования в клиническую практику и создают доказательную базу для его интеграции в систему телестоматологической помощи жителям удалённых, сельских и труднодоступных территорий России.</p></trans-abstract><kwd-group xml:lang="en"><kwd>complete edentulism</kwd><kwd>CAD/CAM</kwd><kwd>complete removable dentures</kwd><kwd>OHIP-14</kwd><kwd>quality of life</kwd><kwd>VAS</kwd><kwd>patient satisfaction</kwd><kwd>digital dentistry</kwd><kwd>teledentistry</kwd><kwd>remote areas</kwd><kwd>randomized controlled trial</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>полное отсутствие зубов</kwd><kwd>CAD/CAM</kwd><kwd>полные съёмные протезы</kwd><kwd>OHIP-14</kwd><kwd>качество жизни</kwd><kwd>ВАШ</kwd><kwd>удовлетворённость пациентов</kwd><kwd>цифровая стоматология</kwd><kwd>телестоматология</kwd><kwd>удалённые территории</kwd><kwd>рандомизированное контролируемое исследование</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Allen F, Locker D.
