An analysis of bony tissue changes during orthopedic treatment of patients with Kennedy IV mandibular defects using dental endosteal implants

Abstract

The recovery of the masticatory function and good esthetic results of orthopedic treatment in patients with Kennedy IV mandibular defects is still a topical problem. The aim of the work was to study biochemical aspects of interactions between bony structures of the front part of the mandible and different endosteal implants, especially under functional load, to develop the most rational method of orthopedic treatment of patients with Kennedy IV mandibular defects. In total, 14 variants of elimination of defects in the front part of the mandible with different supports (natural teeth, standard implants, and miniimplants) for a non-removable orthopedic construction were studied. For each variant, maximal compression and stretch tensions, maximal shift tensions, and the probability of biochemical system destruction were calculated. These variables were compared, and conclusions on the most rational method of orthopedic treatment of patients with Kennedy IV mandibular defects were made; practical recommendations were given.

A numerical study of the tension and movement of dental implants in a sample

Abstract

Three-dimensional models were used to perform a numerical study to define the dependence of the distribution of tension fields and deformations in the implant-bone system on the diameter, length, and the screw thread of the implant. Four kinds of dental titanium implant produced by Konmet, Russia, were studied: 13- or 19-mm implants with or without thread having a diameter of 3.3 or 4.0 mm. In all the variants the abatment was loaded with a vertical load of F = 0.5 kg. The implant movement under two different boundary conditions was minimal when a 19.0-mm implant with a 4.0-mm diameter was used. When the diameter of a screw thread implant was increased by 20%, it decreased maximal implant movement under vertical load by 50%, and maximal tension at the implant apex by 70%. When the length of an implant was increased by 46%, it decreased its maximal movement under vertical load by 40%, and maximal apical tension by 20%. The absence of screw thread in a dental implant increased maximal implant movement under vertical load 1.36 times, and maximal apical tension 1.2 times.

A clinico-laboratory study of the efficacy of removable prosthesis cleaning

Abstract

In an experiment, the effects of various cleaning tablets and SHF irradiation on the adhesion index of the virulent anaerobic agent Prevotella melaninogenica to samples of the polymeric material Villacryl Soft (Zhermack) for elastic and Villacryl H (Zhermack) for hard bases were studied. The study demonstrated a high antiadhesive effectiveness of Corega tablets and SHF waves. A clinical investigation, in which 46 patients with removable prostheses participated, confirmed the efficacy of Corega tablets compared to conventional method of prosthesis care.

Topical diagnostics of ranula

Abstract

In patients with ranula, multispiral computed tomographic sialography followed by multiplane and 3D reconstructions was performed. The results of the analysis gave valuable data on the topographic location of the ranula. This diagnostic technique allows for high precision topical diagnostics of cysts, which is very important for the choice of operative treatment technique.

The tactics of treatment of a patient with a foreign body (a bullet) in the neck (a clinical observation)

Abstract

The authors describe a case of a three month-old blunt bullet wound of the neck. Despite the fact that the presence of the bullet did not manifest clinically, the bullet was removed surgically because it was located closely to a large arterial vessel (according to angiography), which could lead to arterial wall arrosion afterwards. Radiography had not been able to determine whether a surgery was indicated.

A study of morphometric parameters of dendritic cells in the gingival mucosa in chronic catarrhal gingivitis and mild chronic periodontitis

Abstract

The authors studied the numerical density and functional activity of gingival mucosal dendritic cells under normal conditions, in chronic catarrhal gingivitis, and mild chronic periodontitis before and after treatment. According to the study, the number of dendritic cells in the gingival mucosa in periodontal inflammatory diseases was decreased significantly. In long terms after treatment (one, three, and six months) the numeric density of gingival mucosal dendritic cells slowly increased and reached almost normal levels.

Prevalence of borderline mental disorders in the elderly undergoing orthopedic dental treatment

Four hundred and fifty-five elderly dental patients were examined. In 368 (80.5%) patients the following disturbances were revealed: organic anxious disorders (F06.4, 127 cases, 34%); mild organic cognitive disorders (F06.7, 52 cases, 14.1%); organic depressive disorders (F06.32, 31 cases, 8.3%); organic personality disorders (F07.0, 42 cases, 11.3%).

Modern concept of and reflection on complex treatment of periodontal diseases

Abstract

Periodontal pathology is one of the most widely spread one, and its prevalence nowadays is not lower than previously, which proves the inconsistency of the existing so called complex treatment. In everyday practice, the assumed treatment complexes mostly present interpolation of various means of palliative medication, allowing for only short-time effect. In fact, the only procedures from the complex of ones necessary to treat patients with periodontal diseases, performed on patients seeking help are usually removal of deposit and local medication processing. Dentists of different specialties, not to mention physicians, cardiologists, endocrinologists, and even psychologists, do not participate in discussions on management programs. Thus, only a clear program of observational, therapeutic and prophylactic measures will make it possible to fulfill main principles of prevention of periodontal diseases.

The prevalence and structure of extragenital pathology among pregnant women seeking dental aid in the city of Derbent

Abstract

The prevalence and structure of extragenital pathology among pregnant women seeking dental aid was studied. The study found that by the end of pregnancy only 5.63% of women in 2004, 6.21% in 2005, and 5.67% in 2006 did not have a diagnosis of some extragenital pathology. The most frequent problems were iron-deficiency anemia (from 80.63% of cases in 2004 to 86.95% of cases in 2006), thyroid gland pathology (from 14.36% of cases in 2005 to 22.89% of cases in 2004), and urinary tract diseases (from 10.07% of cases in 2006 to 14.08% of cases in 2004). A combination of two or more diseases was revealed in 23.58% of cases in (2005) to 34.68% (2004).

An organizational model of a day-case surgery center in treatment of chronic generalized periodontitis using cryotechnology

Abstract

The authors propose an organizational diagnostics and treatment algorithm for patients with chronic generalized periodontitis using per arterial cryosympathodestruction. They show that treatment of periodontal symptom complex is possible when the principle of dispensary observation is followed, and an integrated computer patient database is built afterwards. An algorithm of treatment and diagnostics is proposed. The method of treatment was developed taking into account chief disturbances underlying the disease, i.e. peripheral blood flow decomposition and vegetative misbalance.

The use of chemiluminescence to study the effects of harmful and hazardous industrial factors on mixed saliva composition in petrochemical industry workers

The article presents results of a study of salivary fluid with superweak irradiation, reflecting the state of non-specific immunity and favoring the manifestation of various unfavorable effects in petrochemical industry workers. The study found that individuals contacting with chemical pollutants displayed substantial oral fluid chemiluminescence.
