The improvement of the method for the evaluation of the bonding strength between the composite coating and the metal

Abstract

The bonding strength between the composite coatings and the adhesive systems used to manufacture metallocomposite dentures is estimated using the method regulated by the ISO 10477 standard that dates to October 1, 2004. The authors propose to register the results of the evaluation by measuring the individual real area of polymer detachment from the metal plate instead of the standard area of the composite coating formed using a special template. According to the authors, the real area of the composite detachment does not coincide with the coating area of the manufactured specimens. This discrepancy should be taken into consideration when recording the results because the bonding strength expressed in megapixels (MPa) is a relative quantity depending on the ratio of the applied force in kilograms to the given area. This area was determined by taking microphotographs of the objects using an optical (light) digital microscope and measuring it with the help of a PlanCad graphic editor. The use of the data on the real area of composite detachment from the metal rather that those on the standard area made it possible to improve the accuracy of measurement of the bonding strength between the composite coating and the metal surface.

Changes of X-ray images, microelement metabolism, and aminoacid composition of feline teeth in periodontitis

Abstract

Adult cats presenting with spontaneously developed periodontitis showed the lower-than-normal content of Ag, Al, Bi, Cr, Mo, and Si in the periodontal bone tissue of both the mandible and the maxilla whereas the level of Co remained practically unaltered and that of Zn significantly decreased. The intraperitoneal administration of 14C-glycine and 35S-methionine resulted in an almost 2- and 1.5-fold increase of incorporation of radioactivity into the periodontal bone protein respectively compared with that observed in healthy animals. Variations of the calculated coefficient reflecting the relationship between the amount of microelements and the relative specific activity (RSA) of the labeled amino acids were less pronounced for Zn than for the remaining seven microelements. It was shown that Zn content in the periodontal bone tissue of the cats with periodontitis was significantly different from that of another microelements, and that this differ were partly attributable to the redistribution patterns of collagen and non-collagenous proteins in the periodontial bone. The fact is proteins of the periodontal bone contain more type 1 collagen in which each third amino acid is represented by glycine whereas the sulfur-containing amino acids are totally absent. In contrast, non-collagenous proteins contain less glycine together with a few percent of sulfur-containing amino acids including methionine.

The functional impact of cytokines of the IL-1 family on the periodontal tissues

Abstract

The understanding of cytokine immunology is of paramount importance for the development of rational methods for the treatment of the periodontal inflammatory diseases. Cytokines belonging to the classical interleukin-1 (IL-1) family, i.e. IL-1-alpha and IL-1-beta along with IL- 18, play a key role in the development of inflammation. Other interleukins of the IL-1 family has recently been identified. They include six cytokines whose genes are localized downstream of the genes for IL-1-alpha and IL-1-beta on chromosome 2 (IL-1F5-10 as well as IL-33, a member of the IL-IR/ Toll-like receptor (TLR) superfamily). IL-1F6, IL-1F8, and IL-1F9 are agonists highly expressed in epithelial cells along with their receptor IL-1Rrp2 which suggests their participation in the protective immune reactions of the skin and the gastrointestinal tract (GIT) including the mouth. Synovial fibroblasts and articular chondrocytes also express IL-1Rrp2 and respond to IL-1F8 which gives evidence of their possible involvement in the development of periodontitis.

Destruction osteoplasty of the mandible using a homoimplant

Abstract

There is a large group of patients presenting with defects of the facial skeleton (including the mandible and maxilla as well as the zygomatic bone) in whom it is practically impossible to perform non-free destruction osteoplasty. We tried to use a bone homoimplant as a substitute for a mobile autograft (an autologous bone fragment) and arrived at the conclusion that this method is highly efficacious and has good prospects for a wider practical application. The experiments were carried out using 7 rabbits in which non-free destruction osteoplasty was performed using frozen homoimplants. It was shown that a single piece of living bone fragment from the jaw is sufficient to initiate the growth of the new bone. The homoimplant used instead of the mobile autogarft proved sufficient to induce the process of osteosynthesis. The quality of the resulting regenerate is described as quite satisfactory.

The efficacy of application of the plasmolifting technique at the stage of orthodontic treatment in the patients presenting with inflammatory-destructive diseases of periodontium

Abstract

The authors analyse the results of the treatment of the patient presenting with inflammatory and destructive periodontal diseases using platelet-rich plasma (PRP) at the stage of orthodontic treatment performed in the broader context of clinical care. It was shown that the application of autogenous platelet-rich plasma promotes natural repair mechanisms and thereby accelerates the healing process. Moreover, this method makes it possible to achieve the two-fold reduction in the frequency of exacerbations of chronic periodontitis. These changes suggest strengthening of the immune system in the oral cavity.

Micronutrient insufficiency in the dietary intake of the patients presenting with generalized periodontitis

Abstract

The results of assessment of the real dietary intake in the patients presenting with generalized periodontitis are reported. The study revealed insufficient supply of their organism with major micronutrients, such as vitamins and microelements, as well as disbalance between these food components. The nutritional disorders documented in the examined patients appear to reflect the relationships between the pathogenetic mechanisms underlying pathological changes in the periodontal tissues and internal organs that include, inter alia, nutrient disbalance.

Abstract

The authors report the results of the comparative analysis of the influence of traditional orthopedic treatment using removable laminar dentures in combination with local estrogen therapy or without it on the prosthetic bed tissues in postmenopausal women; also compared are the time courses of adaptation to the dentures. It was shown that the treatment of women with the use of removable laminar dentures during the postmenopausal period is accompanied by pronounced hypoestrogenism; for this reason, the local application of estrogens significantly enhances the efficacy of therapy compared with the standard orthopedic treatment.

Abstract

We have carried out clinical, stomatological, rheoperiodontological, roentgenological, and index studies that demonstrated the high efficacy of the Vitafon-2 apparatus as well as mexidol, gialudent No3, and octenisept preparations when used to promote rapid normalization of the gingival condition. However, the courses of prophylactic treatment have to be repeated, in accordance with the relevant indications, to prevent the transformation of chronic generalized catarrhal gingivitis (CGCG) to periodonitis and the development of destructive processes in the periodontium.

Modification of the functional and metabolic status of erythrocytes in the patients presenting with chronic periodontitis

Abstract

Certain pharmacological effects of cytoflavin were investigated in the patients presenting with chronic generalized periodontitis. The inclusion of this agent in the combined treatment of the disease leads to a decrease of the severity of the inflammatory process in the periodontal tissues. This effect of cytoflavin is to a large extent attributable to its ability to modulate the functional and metabolic state of erythrocytes. It was shown that cytoflavin promotes restoration of the phospholipid bilayer of the erythrocyte membrane due to the reduction of phospholipase A2 activity. Moreover, the product eliminates effects of oxidative stress.

A total of 50 patients suffering mandibular fractures were available for the observation by clinical and a variety of additional methods. The patients were found to display the activated sympathetic component of their nervous system and the overstressed adaptive mechanisms. The condition of the vegetative nervous system was shown to influence the course of the post-traumatic process.

The assessment of the state of the mandibular bone tissue using the ultrasonometric techniques

Abstract

The measurement of the ultrasound propagation rate in the mega-Hertz and kilo-Hetz ranges through the mandibular bone tissues was performed in 35 subjects with clinically healthy parodontium and in 121 patients presenting with chronic periodontitis. It was shown that the ultrasound propagation rate measured in the mega-Hertz range is 6.8% lower than that measured in the kilo-Hertz range. It is concluded that measurements in the mega-Hertz range allow for a more reliable estimation of the ultrasound propagation rate in the mandibular bone tissues of the patients presenting with chronic periodontitis than measurements in the mega-Hertz range.

The incidence of exacerbations of chronic generalized periodontitis in women of the reproductive age taking into consideration the cyclic changes in the levels of gonadotropic and steroid hormones

Abstract

The present study included 1,329 women of the reproductive age who remained under observation during one year to elucidate the frequency and percentage of exacerbations of chronic generalized periodontitis (CGP) depending on the phase and the stage of the menstrual cycle. The incidence of exacerbations was estimated to be 75.83 per 100 women. The highest frequency of exacerbations was recorded in the late follicular (folliculin---???) and luteal phases of the cycle (40.83 and 34.24 respectively). The largest proportion of exacerbations was also found to occur in the late follicular and luteal phases (80.52% and 95.35% respectively). The results of the study pose new problems to be explored for the better understanding of the mechanisms behind exacerbations of CGP in the women of the reproductive age and for the development of optimal methods for their prevention and treatment.

Abstract

The survey of dental caries prevalence and intensity was carried out among the populations of three high-altitude regions of the Republic of Dagestan. A total of 1,800 subjects (600 in each region) aged between 35 and 74 years were examined. The study has demonstrated a well-apparent tendency toward deterioration of the state of the dento-maxillary system during the period from 2006 to 2008. The data obtained suggest the necessity to develop and introduce the program for the prevention of dento-maxillary diseases among the populations of the high-altitude regions of the Republic of Dagestan.

Abstract

The objective of the present study was to estimate the clinical efficacy of a composite preparation containing potassium fluoride and ecdysteron. A total of 87 patients aged from 21 to 58 years with the increased sensitivity of hard dental tissues were available for the examination. The assessment of clinical efficiency was carried out immediately after the termination of therapy and within the follow up periods of 1, 6, and 12 months. The study has shown that the composite preparation being tested provides an effective tool for the treatment of hyperesthesia of hard dental tissues

Medico-economic expertise of the stomatological care provided in the framework of the compulsory medical insurance (CMI)

Abstract

The medico-economic expertise of the stomatological care is known to most frequently reveal the unfounded overestimation of the volume and the cost of the needed medical care. We have established the variation of this integral parameter between 22.94% in 2006 and 5.06% in 2000. Provision of the low-quality care was documented much less frequently (0.56% and 1.21% in 2006 and 2009 respectively). The performance of medico-economic expertise implies monitoring and analysis of the organization and dynamics, the structure of defects and errors, and the conformity of the therapeutic and diagnostic process with the specified stomatological care consumption coefficient in accordance with the classification of medical services in the framework of the compulsory medical insurance system and the approved standards.

This paper was designed to present the results of the estimation of the stomatological status of the children being brought up in No 1 orphan home, the village of Bogorodskoye, Khabarovsk region. We calculated the indices of dental caries prevalence and intensity, the frequency of periodontal diseases, the prevalence and the structure of dento-maxillary anomalies depending on the age, sex, and ethnic background of the examined children.

The importance of medical documentation for evalution of dental care quality

Abstract

The expert evaluation of primary medical documentation is one of the methods of studying the level of medical care quality. It allows to judge about the correlation between medical care, which is provided to patients with certain type of illness and standards of diagnostic and treatment process. There is a high prevalence of periodontal diseases among the population of Russia. According to researches, the prevalence of inflammatory periodontal diseases increases with the age of the patients (to 81% among 35-44-yr-olds and to 100% among 65-yr-olds), as well as the severity of disease. The present research shows the analysis of the quality of medical documentation filling based on the example of periodontological department of the Moscow dental clinic.
