Evaluation of the relationship between the state of bone tissue in the area of ​​dental implants and systemic and local inflammatory factors

Abstract

Background: detection of proinflammatory cytokines when planning dental treatment can serve as a prognostic criterion in case of risk of occurrence of inflammatory response of the organism to integrated implants.

Aim: to evaluate the relationship between the level of proinflammatory cytokines IL-6, TNF-α, CRP with the condition of jaw bone tissue in the area of installed dental implants taking into account inflammatory phenomena in the peri-implant area.

Materials: the study included 75 patients aged from 27 to 72 years. According to CBCT data, depending on the degree of vertical bone resorption in the area of dental implants, 3 groups of subjects were formed. Group 1 (n=29): resorption less than 1/3 of the implant length; Group 2 (n=32): resorption by 1/2 of the implant length; Group 3 (n=14): resorption more than 2/3 of the implant length. The concentrations of IL-6, TNF-α, CRP in the blood, analysis of the implant hygiene index and macrohistochemical assessment of inflammation in the transgingival part of the abutments were assessed.

Results: the resorption indices and concentrations of proinflammatory cytokines in group 1 were: resorption 0.6±0.1 mm, IL-6 0.34±0.31 pg/ml, TNF-α 0.5±0.1 pg/ml, CRP 2.6±6 pg/ml; in group 2: resorption 2.4±0.8 mm, IL-6 1.3±0.5 pg/ml, TNF-α 1.7±0.5 pg/ml, CRP 3.45±3.4 pg/ml; in group 3: resorption 4.6±1.6 mm, IL-6 2.3±0.4 pg/ml, TNF-α 2.3±0.2 pg/ml, CRP 3±2.2 pg/ml.

Conclusion: the degree of bone resorption in the area of implants has a direct correlation with the level of pro-inflammatory cytokines IL-6, TNF-α. The results obtained may indicate the importance of assessing the level of cytokines IL-6, TNF-α in identifying the risk group of patients who have had implants under load for 5 years or more, taking into account the local manifestation of inflammation in the area of the transgingival part of the abutments. In addition, cytokine status can be recorded during implantation planning, which can be a predictive factor in predicting further implant stability.
