Analysis and evaluation of the clinical efficacy of treatment of chronic periodontitis using stromal vascular fraction and native platelet autoplasm: A controlled randomized clinical study

Abstract

Periodontitis is one of the most common dental diseases, polyetiology, is the cause of constant discomfort in the oral cavity, in the form of inflammation and sensitivity of the gums, limiting physiological needs, such as eating and communication. Despite the availability of various treatment methods (conservative, surgical, physiotherapeutic), the need for new, effective methods remains relevant.

 Autoplasma-gum plasmolifting has been widely used in dentistry for more than 20 years, but the stromal-vascular fraction is just beginning its journey. Why not combine two safe regenerative masses into one, achieving a quick effect with long-lasting remission?

 We treated 85 patients aged 36 to 65 with chronic periodontitis (CP). The patients were divided into 2 groups depending on the type of treatment: in the I-main group (OG), 43 patients with CH were injected in addition to standard treatment a mixture of stromal-vascular fraction (SVF) and native platelet autoplasm (TAP) was injected into the inflamed interdental papillae in the II control group (CG) of 42 patients with CP who received basic treatment. All patients underwent a dental examination, and a periodontal chart was compiled, which included the following indicators: the depth of periodontal pockets, pathological tooth mobility, bone tissue resorption, a computed tomography scan, the Muller bleeding index and hygiene index, and the condition of periodontal tissue microcirculation. Results. After performing SRF and TAP in the main group, all indicators improved, namely, the depth of periodontal pockets decreased by 1.4 mm, bone tissue resorption by 25.5%, tooth mobility decreased by degree, increased the volume of blood flow perfusion, reaching the norm of 30.02±4.36 perf. units, the values of the indices also showed themselves in a better way. Conclusion. When treating the method of SRF and TAP, we achieved a stable remission, restoration of the periodontal attachment and the level of bone tissue mineralization in the interdental septa of the alveolar bones.

BACKGROUND Periodontitis is one of the most common dental diseases, polyethological, it causes constant discomfort in the oral cavity, in the form of inflammation and sensitivity of the gums, limiting physiological needs such as eating and socializing. Despite the availability of various treatment methods (conservative, surgical, physiotherapy), the need to find new methods remains relevant.

AIM Development of a method for the treatment of chronic periodontitis using stromal vascular fraction and native platelet autoplasm to improve the effectiveness of treatment.

METHODS The inclusion criteria were persons with a BMI> 30, over 36 years of age, and of any gender.

Exclusion criteria: pregnancy, chronic diseases, BMI<30, neoplasms, infectious and mental diseases, diabetes mellitus, hypertension.

An outpatient appointment was conducted in a dental clinic, with a frequency of 1,3,6 months.

The target indicators were: 1- the Mulleman hygiene and bleeding index, assessed by staining, 2-the depth of the periodontal pocket, measured with a periodontal probe, 3- pathological tooth mobility using tweezers, 4-bone resorption, studied on a CT scan, 5-the microcirculation of periodontal tissues was assessed on a laser analyzer of capillary blood flow LACC-0P".

RESULTS: Chronic periodontitis (CP) was treated (K 05.3) 85 people from 36 to 65 years old.

The patients were randomly divided into 2 groups depending on the type of treatment:

I was the main group of 43 patients with CP who underwent injection of stromal vascular fraction (SVF) and native platelet autoplasm (TAP) into periodontal tissue in addition to standard treatment.

II was the control group of 42 patients with CST who received basic treatment therapy.

In the main group, who were injected with SVF and TAP, the indices and additional research methods showed the following indicators: hygiene index (IG) 3.5±0.021, bleeding index (IR) = 3.6±0.022, depth of periodontal pockets (GC) 5.1±2.220, dental computed tomography: resorption of the alveolar bone to 1/3 of the root length. During Dopplerography, the volume of blood flow perfusion (OK) is 19.01±0.034 (perfect units), the rate of blood flow perfusion (SK) is 1.62±0.034 (perfect units). In 2 groups receiving basic therapy, the indicators were: IG 3.4± 0.023, IC index 3.5±0.021, GC 5.0±2.130, dental computed tomography: resorption of the alveolar bone to 1/3 of the root length. During Dopplerography, OK 18.05±0.033 (perfect units), SK 1.54±0.028(perfect units)/

CONCLUSION The results of treatment of group 1 patients showed a significant improvement in dental status. Clinically, there are improvements in the condition of the gums, a decrease in bleeding and swelling of the marginal and papillary gums. The bone tissue of the alveolar processes has stabilized, most of the pockets have closed, further bone resorption has stopped, and there is no tooth mobility
