Evaluation of operator error in determining craniometric landmarks and calculating radiological anatomical indicators of the temporomandibular joint: cross-sectional study

ABSTRACT

Background. One of the key tasks of modern dentistry is the analysis of X-ray images of the temporomandibular joint (TMJ). Errors that occur at the stage of analysis of data cone-beam computed tomography (CBCT) TMJ, are the reason for the formulation of incorrect radiological conclusion, inadequate planning of treatment.

Purpose. To evaluate the degree of influence of the operator error on the formulation of radiological conclusion about the condition of TMJ according to CBCT.

Materials and methods. A sample of 20 CLCCT scans of the facial skull bones was taken from 20 patients with TMJ disorders aged 25 to 64 years (14 women and 6 men). To determine the extent of operator error in the analysis of CLCCT scans of the TMJ, a unified software and a protocol for determining cranimetric points were used to calculate the anatomical and topographic position of the mandibular head. The analysis of CLC was carried out using the method of automated craniometry of the anatomical structures of the skull that we developed (certificate of state registration of computer programs No. 2017662860). The deviations of the calculations were analyzed from the obtained data.

Results. The study revealed significant systematic and random errors in the estimates of the analyzed cranimetric indicators, which is unacceptable for clinical practice.

Conclusion. To reduce the impact of operator error on the formulation of an X-ray report when analyzing CLCCT of the facial skull bones, along with the specialist's theoretical training, a significant portion of the training program should be dedicated to on-the-job training under the guidance of an experienced mentor.
