Lepilin A.V., Venig S.B., Lyasnikova A.V., Zakharevich A.M., Smirnov D.A., Mostovaya O.S., Lepilin A.V., Venig S.B., Lyasnikova A.V., Zakharevich A.M., Smirnov D.A., Mostovaya O.S.

Abstract

At present, it is widely believed that endosseous implantation into the fresh extraction sites is the method of choice in every relevant clinical situation. Given the risk of infectious complications associated with peri-radicular lesions, the local antimicrobial treatment is indicated. A dental implant with the silver-containing biocomposite coating has been developed. The in vitro study has been carried out to elucidate morphological and chemical properties of the surface of the new dental implant. In vivo experiments have demonstrated osteointegration of its internal structure with the biocomposite antibacterial coating.

The assessment of treatment efficacy in patients presenting with chronic generalized periodontitis from the content of phosphatidylinosites and their metabolites in the gingival blood

Zorina O.A., Kulakov A.A., Boriskina O.A., Zorina O.A., Kulakov A.A., Boriskina O.A.

Abstract

The authors consider the use of varying biochemical characteristics of phosphatidylinositol metabolism for the objective control of the efficacy of the treatment of patients presenting with chronic generalized periodontitis. The following trends have been revealed in the post-treatment period: normalization of phosphatidylinositol levels in the gingival blood suggesting cessation of the injurious action of aggressive factors on the periodontal tissues; recovery of the initial level of arachidonic acid that reflected reduction in the severity of the inflammatory process; normalization of the prostaglandin E2 content indicating reduced destruction of the bone tissue. It is concluded that comprehensive examination of the gingival blood from the patients presenting with chronic generalized periodontitis is the sole reliable approach to the assessment of the severity of the disease and the observation of dynamics of periodontal condition during treatment.

Abstract

The objective of the present work was to evaluate the state of marginal periodontal tissues after the treatment of the interdental surface of restorations from light-cure composite resins using different finishing strips. A total of 125 direct restorations on interproximal tooth surfaces were available for the clinical evaluation. It has been shown that the method of finishing the contact surface of restorations has an appreciable effect on the hygienic characteristics and PMA index of the interdental region. The best results were obtained in the group of patients treated with the use of plastic strips having four different degrees of abrasivity.

Morphological, morphometric, and functional characteristics of melatonin-producing gingival mast cells in patients presenting with inflammatory periodontal diseases and dynamics of their post-treatment changes

Masumova V.V., Bulkina N.V., Osipova Y.L., Savina E.A., Masumova V.V., Bulkina N.V., Osipova Y.L., Savina E.A.

Abstract

The objective of this work was to evaluate quantitative density and functional activity of melatonin-producing mast cells of normal gingival mucosa obtained from the patients presenting with inflammatory periodontal diseases before and after their treatment. The study has demonstrated a significant decrease in the number of mast cells producing melatonin in gingival mucosa during inflammation of parodontium. Long-term follow-up observations (1, 3, and 6 months after the treatment) revealed a steady rise in the quantitative density of melatonin-producing gingival mast cells to a practically normal level in the patients with chronic generalized catarrhal gingivitis (CGCG) and mild chronic generalized periodontitis (mCGP). At the same time, this parameter remained much lower than the normal one in the patients presenting with moderate chronic generalized periodontitis (moCGP) and severe chronic generalized periodontitis (sCGP). Its value correlated with the degree of inflammatory and destructive changes in the periodontal tissues.

Tlustenko V.P., Gil'miyarova F.N., Golovina E.S., Tlustenko V.S., Kuznetsova E.A., Tlustenko V.P., Gil'miyarova F.N., Golovina E.S., Tlustenko V.S., Kuznetsova E.A.

Abstract

This paper is devoted to the biochemical research methods with special reference to their relevance and non-invasivity. The authors present the reference values for metabolic parameters of oral fluids (total protein, albumin, and calcium content) from the subjects of the control group without somatic and stomatologic pathology and from those having dental implants in the oral cavity. Comparison of normal and pathologically altered values provides a basis for the assessment of the risk of developing dental periimplantitis.

Abstract

The species composition of type I pathogenic periodontal microflora was identified by the multiplex PCR technique. The study revealed the presence of Porphyromonas gingivalis, Prevotella intermedia, Aggregatibacter actinomycetemcomitans (Actinobacillus actinomycetemcomitans), Tannerella forsythia (Bacteroides forsythus), and Treponema denticola. The samples were taken from 300 periodontal pockets localized in symmetric regions of the oral cavity of 75 patients presenting with chronic generalized periodontitis. It was shown that 1/5 of the patients had type I paradontopathogenic bacteria in the examined regions; they were absent in 1/4 of the patients. The pigment-producing bacterial species (P. intermedia and P. gingivalis) could be found in 3 or 4 quadrants of the dentogingival sulcus with a probability of 50-70% whereas T. denticola and A. actinomycetemcomitan occurred only in one quadrant in 47-56% of the cases. The data obtained can be used to estimate the role of local and systemic etiological factors in the susceptibility of the organism and their contribution to the development of various pathological conditions. Moreover, they are of importance for the clinical diagnostics and treatment of inflammatory diseases of the oral cavity.

Clinical and organizational aspects of the development of maxillary sinus perforation in the patients undergoing tooth extraction in polyclinic departments

Shchipskiy A.V., Mukhin P.N., Kurbatova A.S., Shchipsky A.V., Mukhin P.N., Kurbatova A.S.

Abstract

Analysis of the results of examination and treatment of 103 patients presenting with oroanthral fistula admitted to a maxillofacial hospital has demonstrated the predominance of working-age subjects with this pathology in Moscow state-owned clinics and their unwillingness to correct perforation of the maxillary sinus immediately after tooth extraction. Diagnostic errors are supposed to be the main cause of perforation. The presence of "artifacts" in the maxillary sinus (such as tooth roots, rubber drains, and pharmaceutical products) give evidence of inadequate treatment. The authors emphasize the importance of updating the educational and methodological programs for the advanced training of stomatologists. Special attention should be given to the improvement of their practical skills in correcting maxillary sinus perforation during the regular outpatients' visits.
