METHODS FOR DETERMINING THE DEGREE OF ABRASIVE WEAR OF DENTAL MATERIALS

Abstract

One of the main criteria for the quality of dental treatment is to obtain a long-term result. The problem of achieving such a result becomes especially acute in the treatment of such pathology as, for example, increased tooth wear, where a stable result is possible only with the use of high-strength materials resistant to abrasive wear. This article presents two methods for objectively assessing the degree of erasure of materials that can be used both to control the quality of the treatment performed and to assess the quality of new materials. Since the manifestation of abrasive wear of materials is an increase in the area of erasing facets, both methods are based on estimating changes in the area of occlusal contacts over time. The first proposed method allows us to estimate changes in the area of occlusal contacts of the teeth using digital occlusiograms. Patients underwent occlusion using the TScan III device after 1 month and 24 months after the restoration of the occlusal surfaces of all teeth of one dentition, then the area of occlusal contacts was measured. The second method allows to exclude the influence on the results of the study of such factors as the tone of the chewing muscles, the frequency and nature of the food used, the features of the occlusal relationships of the teeth, etc. To this end, the restorations from the studied material were installed on the chewing surfaces of all the teeth of the upper jaw model of the “Stand of chewing movements”. Next, wax occlusiograms were made immediately after the installation of the restorations and after 1 million chewing movements, and the area of the occlusal contacts was determined. In both cases, evaluating the change in the area of occlusal contacts, they made a conclusion about the degree of abrasive wear of the material. The use of digital and wax occlusiometry is indicative of determining the area of occlusal contacts when assessing the degree of abrasive wear of dental restorative materials.

Abstract

The aim of current work was study of spreading chlorine photosentitizer in tooth tissues after its injection root canal. Study was held with help of local fluorescent spectroscopy method. Laser radiation with 408 nm letgth was used for stimulation of exogenic fluorescence of photosentitizer. After injection photosentitizer in root canal good spreading in microchannels and the whole volume of tooth was shown.

CLINICAL AND X-RAY STRUCTURIZATION OF THE ENDOPERIAPICALLY STATUS OF THE EARLIER TREATED TEETH IN AESTHETIC SIGNIFICANT ZONE

Abstract

In article are presented results of endocanal treatment of 87 function-oriented frontal groups teeth having centre of periapically destruction in the manner of resorbtion of the bone structures at the age from 20 to 50 years and senior. In depending of periapically resorbted diameter patients were divided into 3 groups: I group - 45 patients beside which discovered centers of periapically destruction by diameter from 1 to 3 mm; II group - 19 patients with centre bone resorbtion of periapically location by diameter from 3 to 5 mm; III group - 14 patients with periapically centre of destruction by diameter more than 5 mm. Reliable reduction of the sizes centre of periapically resorbtion beside patient of 1st main group existed in 12 months after endodontic treatment. Under full-fledged endodontic treatment area of the centre destruction in periapically tissue decreased in 4.8 times and reconstruction bone fabrics in the form of the decreases area of resorbtion centre exists in 60,0± 3,5%.

EVALUATION OF CLINICAL ASPECTS OF APPEARANCE OF TEETH NONCARIOUS DEFECTS IN BROXISM PATIENTS

Abstract

Purpose - to study the etiology, prevalence, clinical course, functional examination and treatment of non-carious lesions located in the cervical area of patients with bruxism. Materials and methods. A prospective study of 200 outpatient cards of dental patients at the dental clinic of the Federal State Budgetary Educational Institution of Higher Medical Education of the KMGU of the Ministry of Health of Russia (Kursk) and Federal State Budgetary Educational Institution of Higher Medical Care of the Kuban State Medical University of the Ministry of Health of Russia (Krasnodar) in the age range from 18 to 55 years, whose average age was 38 ± 6.7 years. We studied the localization of cervical defects, etiological factors of occurrence, and also conducted an electromyographic study of the chewing muscles at rest, when the jaws were compressed and when chewing. Results. When studying the localization of cervical defects in different functional groups of teeth, defects in the cervical area of premolars were noted in 41 patients - 57%, in 18 - on canines (25%), in 10 - on incisors (13.8%), in 3 - on molars (4.2%). It was revealed that the trigger mechanism was a number of factors: partial loss of teeth (20 patients were diagnosed with partial absence of teeth of both jaws - Kennedy class III-IV without correct orthopedic treatment), 17 respondents had flattened occlusal surfaces of teeth after therapeutic treatment, 31 were diagnosed abrasion of varying severity and localization, in all patients - a violation of occlusal relationships. In 56 patients (77.77 %) a combination of several etiological factors was noted. A comparative analysis of EMG indices of paired masticatory muscles in the studied group showed that before the start of dental treatment, patients had uncontrolled bursts of bioelectric activity (BEA) in opposite muscles from opposite sides, an increase in the amplitude of muscle contractions during compression in both muscles (mostly in actually chewing ), decrease in the amplitude of muscle contractions during compression due to the rapid fatigability of muscles at rest. Conclusion. Thus, questions of etiology are still open and subject to further study. Of the many known causes, the most common are violation of occlusal relationships, muscular parafunctions, external traumatic factors, etc. The appearance of bruxism is associated with a significant risk of disturbing the occlusal relationship with a subsequent increase in the load on the tooth enamel. In this regard, a rational comprehensive dental treatment of patients with the studied combined pathology should be carried out after conducting electromyographic studies with samples of “rest”, “compression” and “chewing” strictly in accordance with the indications and timing of dynamic observation at least 1 time in 3 months.

THE STRUCTURE OF MALOCCLUSION AMONG SCHOOL CHILDREN OF THE MODERN CITY

Abstract

Physiological occlusion is a type of occlusion in which all parts of the masticatory apparatus harmoniously interact. The pathology of occlusion is in violation of this balance. There are many factors that directly affect the occlusion - as a general organization (heredity, the influence of the endocrine system, etc.), and local factors (trauma to the oral cavity, harmful habits). In view of this, the prevalence of various nosological forms of dental-maxillary anomalies (hereinafter referred to as AFC) is a relevant field for research in modern dentistry. After analyzing the scientific works of a number of authors [1-5], we conclude that the ASC often has a combined nature (a combination of different forms of pathologies). Many studies conducted among various populations provide a variegated picture of the distribution of this or that anomaly. So, the analysis of the author, conducted by Hardy et al. [6] showed that the distribution of class III anomalies varies from 0 to 26.7%, depending on the population studied. Therefore, knowledge of the epidemiological situation and the correct definition of a particular form of the disease is crucial in the further planning of treatment tactics.

LABOR AND FINANCIAL COSTS IN TREATMENT OF DENTOALVEOLAR ANOMALIES IN THE PERIOD OF THE MIXED OCCLUSION

Abstract

The aim of the present study is to analyze the complexity and cost of modern methods of orthodontic treatment of children during the period of replacement bite. The study of the duration and frequency of visits to the doctor the orthodontist with the use of technology 2x4, removable plates with artificial teeth, ring spacer, plate from the front Nakonechny pad and plate valve for the language of the Twin-block appliance, mask dilara combined with the expanding apparatus, plate screw, machine screw hyrax expansion. Based on the complexity of the calculated salary orthodontist doctor, medical and other personnel. The cost of consumables at the clinical and dental stages was determined by taking into account the purchase prices and the cost of a specific manipulation by weighing or measuring. Calculated the expenditure on maintenance of workplaces of the doctor and dental technician in the calculations of the required funding early orthodontic treatment used more information about the structure and prevalence of orthodontic pathology in children in the period of the mixed occlusion. It was found that the complexity of modern orthodontic treatment during the period of replacement bite varies from 5.55 hours (spacer with a ring in case of premature loss of a temporary tooth) to 10.73 hours in the treatment of distal bite with the twin-block device. The cost of orthodontic treatment during the period of replacement bite ranges from 12.6 thousand rubles (spacer with a ring in case of premature loss of a temporary tooth) to 33.4 thousand rubles in the treatment of mesial bite using a dilyar mask in combination with an expanding apparatus. Taking into account the prevalence of dentoalveolar anomalies in children during the period of replacement bite, the necessary financial costs for orthodontic treatment per one examined child and per child with a dentoalveolar anomaly reach 30 and 40 thousand rubles, respectively.

SYNTHETIC LACTOFERRIN IN DENTAL DISEASES PREVENTION AND TREATMENT: CURRENT TRENDS AND PERSPECTIVES

Abstract

The current trend in dental science is to develop novel approaches to the treatment of dental diseases. More often the body’s own proteins are being used, as well as lactoferrin that is the component of exocrine glands secretions. At present lactoferrin can be obtained biotechnologically. Lactoferrin is one of the innate immune system components and is a multifunctional glycoprotein from the transferrin family. This protein is capable of iron binding and thus deprives the microbiota of essential microelement. It’s efficacy against Str. mutans, A. actinomycetemcomitans, P. Gingivalis, P. Intermedia, Candida albicans was demonstrated. Lactoferrin is active against cytomegalovirus, herpes simplex virus, human immunodeficiency virus, hepatitis B and C virus. Immunomodulatory possibilities of lactoferrin have been shown. In dentistry recombinant lactoferrin is successfully applied in treatment of periodontal diseases, oral mucosa lesions of bacterial, viral and fungal etiology and also in dental caries prevention.
