Improving the disinfection of lateral microcanals in endodontic dental treatment

Abstract

We studied the penetration depth of an antiseptic introduced in the dentin of the tooth by using various methods. An in vitro experiment was performed to determine effective parameters of ultraphonophoresis and electrophoresis, providing the maximum-diffusion antiseptic Decasan in the dentin of the tooth. Compared with the control, three physiotherapy sessions of the root canal significantly increased in-depth penetration in single-rooted teeth. The inclusion of Decasan in the endodontic treatment of patients with chronic apical periodontitis for drug processing the root canal irrigation system is effective because it promotes deep diffusion of antiseptic solution in dentinal tubules.

Effectiveness of kinesioteping as an additional method of treatment in patients in the rehabilitation period after osteosynthesis of the lower jaw

Abstract

Background. The article describes the application of kinesioteiping as an additional treatment for patients in the rehabilitation period after osteosynthesis of the lower jaw. Aims. To evaluate the effectiveness of kinesiotherapy in patients after osteosynthesis of the lower jaw in the rehabilitation period. Materials and methods. The nociceptive sensitivity and edema size in patients were evaluated after osteosynthesis of the lower jaw, and the durations of the rehabilitation period with and without kinesiotherapy were compared. The study involved 30 patients who were hospitalized (from January 2019 to February 2020) in the Department of Maxillofacial Surgery, Regional Clinical Hospital, Ryazan. A visual analog pain scale and a photographic protocol were used to evaluate the effectiveness of the methodology. Results. On the third day of the study, 46.6% of the patients in the control group expressed an improvement in their health. Subjectively: mouth opening restrictions of 2 cm were observed, spontaneous pain in the lower jaw was preserved, and soft tissue edema was preserved. In the study group, the results were more positive compared with the first day after surgery. On the third day, 66.6% of the patients recovered better and faster. Subjectively: the level of pain in the lower jaw decreased, soft tissue edema reduced, and the mouth opening restriction of 2 cm was preserved. Conclusion. Kinesiotherapy is an effective and well-proven additional non-drug method that allows to reduce the rehabilitation period after osteosynthesis of the lower jaw.

Comparative assessment of the preventive efficacy of Individual oral hygiene products in persons with decompensated caries

Abstract

Background. The violation of the marginal tightness of composite restorations and the recurrence of the carious process remains relevant to this day, especially in persons with a decompensated form of caries activity. In this regard, of undoubted interest is the possibility of a wide preventive effect of individual oral hygiene products with a remineralizing effect, particularly those containing brushite crystals. Brushite crystals have unique adhesive properties and tropism to hydroxyapatite crystals on the enamel surface. The principal mechanism of action of the “RemarsGel” system is a chemical reaction that occurs when calcium nitrate from tube # 1 is mixed with ammonium hydrogen phosphate from tube # 2, as a result of which a brushite crystal is formed on the surface of the teeth. Material and methods. From a clinical point of view, this study compared the clinical efficacies of a standard prophylaxis program (using traditional fluoride-containing personal oral hygiene products) of carious recurrence in patients with a high degree of caries activity and a developed program for using a natural two-component complex to strengthen and remineralize RemarsGel enamel. The effectiveness of the applied technique was monitored using clinical research methods immediately after sanitation and 1, 12, and 36 months after it. Results. The high efficiency of brushite crystals was established in terms of preventing violations of the integrity of the enamel–composite joint, which has a direct and immediate effect on the recurrence of the carious process. A statistically significant difference in the Ryge criteria was found between group Nos. 1 and 2. These results were confirmed by scanning electron microscopy and statistical processing of the research results. Conclusion. A scientifically based indication of the intended use of the system must be added to the list of main indications for use — in the presence of a significant amount of adhesive restorations in the oral cavity, made using direct and indirect methods, especially with decompensate caries.

Effect of result retention on patient satisfaction with orthodontic treatment

Abstract

Background. Nowadays, the number of orthodontic care requests has increased significantly. The technical capabilities of orthodontics have improved exponentially with technological progress. The necessary care can now be provided at almost any age, and the need for aesthetic results has increased, as motivated by the mass media. As a result, patients have high demands to orthodontists. New technologies, such as cortical support, can solve dental abnormalities that previously could only be cured by surgery. However, maintaining high-quality results remains a challenge. Materials and methods. The objective parameters of orthodontic treatment were analyzed and compared with the subjective assessment of patients. Conclusion. The patient’s motivation, the desire to cooperate with the doctor, and a clear understanding of the treatment goals enable to obtain and retain high-quality results for a sufficiently long period of time.

Optimization of healing in patients with odontogenic phlegmon of the maxillofacial region

Abstract

Aim. This study aims to increase the efficiency of the wound process in patients with odontogenic phlegmons of the maxillofacial region by using energotropic and antioxidant agents. Material and methods. The clinical study included 42 patients with odontogenic phlegmons of the maxillofacial region. The patients were divided into two groups: comparison group (patients received traditional treatment) and main group (patients received treatment with Cytoflavin in the treatment regimen). Results. In the main group, the appearance of the first granulations occurred 2.1 times faster (on average on the 3rd day) in comparison with the granulation tissue in patients of the comparison group. Signs of edge epithelialization of wounds appeared almost 3 days earlier (on average on the 6th day) in the main group than in the comparison group; on the 7th day, secondary sutures were already applied, which was 1.6 times faster than the same parameter group 2, clearly suggesting the acceleration of reparative processes. Conclusion. The dynamics of clinical manifestations indicates the comparative effectiveness of using Cytoflavin as part of complex therapy. This finding is confirmed by the accelerated terms of wound healing, the appearance of granulations, epithelialization of the wound edges, and the imposition of secondary sutures.

Abstract

This study analyzed the role of Helicobacter pylori in the development of lesions of the oral mucosa and explored the relationship between the prevalence and intensity of periodontal disease and the stage, duration, and severity of gastrointestinal diseases associated with H. pylori infection. The newly identified factors of H. pylori pathogenicity that participate in the development of lesions of the oral mucosa in case of a body infection were also determined.

Methodological recommendations for improving the adaptability of patients with accessed defects of the maxillofacial region

Abstract

Introduction. After the application of a removable prosthesis, adaptation is one of the most difficult periods for the patient. This period is especially pronounced in the absence of experience in using a removable prosthesis or in the presence of general somatic pathology in parallel. Acquired defects of the maxillofacial area require restoration of anatomical areas of significant volume, and the problem of adaptation to replacement constructions has become significant. The adaptive ability of the patient and oral stereognosy are closely related. Tongue muscles stimulate brain activity to improve the adaptability to removable structures. In this study, a set of special adaptive exercises was developed to increase the synthetic activity of the cerebral cortex and improve adaptability to removable prostheses. Materials and methods. The study involved patients aged 44–75 years. All patients were divided into three groups: clinical control (30 people), group with acquired defects of the dentition of the maxilla (25 people), and group with acquired defects of the upper jaw after surgery (25 people). Patients in the clinical comparison group were scheduled for surgical treatment of the teeth and the imposition of immediate prostheses immediately after surgery. Immediate prostheses were made prior to surgery using a standard technique. Results. Patients of the main group with neoplasms of the maxillofacial region were planned to resect the maxilla and apply a resection prosthesis directly on the operating table. Manufacturing of resection shaping prostheses was carried out following the standard procedure. All patients underwent stereognostic testing, and the degree of the patient’s adaptive ability was determined. Depending on the assigned adaptive support, each group was divided into subgroups. Patients in the first subgroup were prescribed modernized functional training, and patients in the second subgroup received the main treatment without prescribing exercises. Conclusion. On the basis of the results of examination, treatment, and dynamic observation of groups of patients with acquired defects in the dentition, the adaptation support reduced the time of adaptation to new removable dentures. The results were confirmed by objective research through the stereognostic testing and subjective feelings of patients.
