COMPARATIVE EVALUATION OF ADHESION OF CEMENTS FOR PIN DESIGNS TO THE SURFACE OF DENTIN AFTER ROOT CANAL TEMPORARY FILLING TOOTH OSTEOTROPIC MATERIAL

Abstract

This publication presents the results of laboratory studies of the effects of temporary filling material based on hydroxyapatite and tricalcium phosphate on the adhesion in the root canal pins and pinlay core, fixed on the composite glass ionomer cements. It was revealed that a temporary application of paste in the channel does not affect the quality of the attachment strength of prosthetic used in post endodontic restoration of the tooth.

BIOMECHANICAL CHARACTERISITICS OF PULP REMOVA IN CASES OF PERIODONTAL DISASES

Abstract

The state of extensive deformation of ‘tooth-parodont’ system has been explored considering the stage of the alveolar crest bone resorption and anatomical consequences of tooth depulpation. The research involving mathematical simulation in combination with the finite element analysis and Schleicher-Nadai theory has stated that removal of single-rooted teeth caused by periodontal diseases does not increase the maximum allowable level of occlusal load in case of good bone mineralization. Provided depulpation is inevitable, periodontal tissue should either not be substituted with fibrious tissue or replaced with one third intra alveolar length. Biomechanical features imply a varied choice of methods and tooth filling materials used for filling root canals as well as revision of parodontogram rates in terms of periodont changes after pulp removal.

СHANGE OF THE MICROSTRUCTURE OF ENAMEL AND DENTIN UNDER THE INFLUENCE OF THE ROTARY TOOL IN THE TREATMENT OF CARIES (IN VITRO STUDY)

Abstract

At the present stage of development of the basic tool for cutting of necrotic tissues and forming, a cavity in the treatment of dental caries remains a rotary tool. Since the main dental restoration technique is adhesive technology, the quality of the enamel and dentin adhesive surfaces plays a decisive role in the future as a treatment. In vitro, using scanning electron microscopy was studied impact complex of hard teeth tissues preparation components (type cutting tool drive, feed rate, etc.) on the microstructure of adhesive enamel and dentin surfaces. Modeled and rendered the most frequently committed errors by doctors at preparing dental hard tissues. Diamond tool in combination with the turbine drive has the most adverse effect on the microstructure of dentin. Inadequate selection of tungsten carbide tools leads to significant destruction of enamel prisms, which in turn is responsible for violations of the adhesion of filling materials and recurrent caries. Just complete an adequate combination of the above components of hard teeth tissues preparation provides the necessary condition of the adhesive surfaces and high-grade quality of the composite adhesion.

PATHOLOGIC FEATURES OF PERIODONTAL TISSUES IN EXPERIMENTAL OSTEOPOROSIS

Abstract

The article presents information about some pathological changes in periodontal tissues during experimental osteoporosis. The experiments were conducted on 8 three-year sheep, which have at different times after ovariectomy removed dentoalveolar segments in the region of incisors of the lower jaw. Pathological changes in periodontal animals in experimental osteoporosis were local manifestations of the general condition of the body that are typical of acute hypoxia mixed character. It was found that experimental osteoporosis in periodontal develop processes accompanied by acute circulatory disorders, which worsen the conditions of metabolism, which ultimately leads to destructive changes of argyrophilic and collagen fibrils, as well as individual nerve fibers. Studies have shown that these effects are reversible, and after 120 days to the fore the phenomenon of compensation in periodontal starting to recover basic histological structure. Prolonged hypoxia, as the main link in the pathogenesis of degenerative and inflammatory of forms of periodontal disease may be modified by the proven of efficiency pharmacological means. The detected changes at the osteoporosis models can be the basis for further study of the role of autologous mesenchymal stem cells and materials-matrices of various origins in optimizing the osseointegration process.

Abstract

The authors have studied the steroid hormones in the saliva of patients with hypogonadism who had concomitant sialadens. It is established that sialadens has an impact on preanalytical phase, a further decrease on the background of treatment. The calculated values of concentrations of steroid hormones in saliva in normal and pathological hypogonadism, developed diagnostic criteria based on indicators of testosterone saliva-tion and viscosity of saliva.

During long-term orthodontic treatment there are various complications associated with a reduction in the level of hygiene. The article is devoted to the study of the hygienic condition of the oral cavity and the level of dental education in dynamics in patients undergoing orthodontic treatment. The examination and questioning of 40 patients aged from 18 to 35 years with fixed orthodontic appliances. In patients after 12 months of use braces deteriorating hygienic condition of the oral cavity. Care should be taken to inform patients about the care products and hygiene items, and their rational use, depending on the timing of orthodontic treatment and dental status.

OPTIMIZATION OF TREATMENT OF DESTRUCTIVE FORMS OF CHRONIC APICAL PERIODONTITIS

Abstract

Increase of efficiency of treatment of patients with chronic apical periodontitis is a priority in dentistry. A root canal is a breeding ground for microorganisms. The main reason for unfavourable treatment outcomes of teeth with chronic apical periodontitis is incomplete debridement of the root canal system. Optimizing the treatment of patients with chronic periodontitis with the use of complex drugs galodent No. 4 (clotrimazole, tracert and calcium hydroxide) is a priority in dentistry. As a result of researches it is established that the antibacterial effect of the temporary investment of complex drugs galodent No. 4 and clotrimazole in the root canals of teeth with chronic apical periodontitis is 2,9 times higher and the proportion of positive results is, on average, 1,5 times higher compared to the practiced methods in the comparison groups

CLINICAL FOUNDATION FOR USING REMEDIES WITH NANOHYDROXYAPATITE AND FTOR IN TREATMENT ENAMEL CARIES STAGE OF WHITE SPOT LESION

Abstract

The present article is devoted to the study of clinical effectiveness of remineralizating agents on the basis of hydroxyapatite created by the departments of Pharmacy and Dentistry of ESBEI HE “Kuban State Medical University, Ministry of Health Care of Russia”. The new remedy remineralizating effect was evaluated in treating patients with enamel caries (stage of white spot) and compared with the drug “ Ftor Varnish” (produced by “Omega Dent”). Clinical efficacy was studied by TER-method, the vital staining of the teeth and a laser fluorescence of hard dental tissues with the “DiagnoDent” device. The results obtained have proved the remineralizing effect after gel-based hydroxyapatite application, which allows to recommend the proposed gel for remineralization of hard dental tissues for caries prevention and treatment in the stage of white spot. Therapeutic and prophylactic remedy was made in the form of white translucent gel, which eliminates the undesirable staining of the hard tissues, is easy to apply in a thin layer on the surface and fills microdefects

USE OF ERBIUM LASER FOR SURGICAL TREATMENT OF ROOT CYSTS OF THE JAWS

Abstract

The article is devoted to investigate the efficacy of surgical treatment with Er:YAG laser of patients with root jaw cysts. An examination and surgical treatment of 55 patients with root jaw cysts of different localization were carried out: 29 patients were undertaken the traditional method of cystectomy, 16 of them - in combination with the using of “Osteodet”. In 26 patients we held laser cystectomy with application of Er:YAG laser, in 16 cases also “Osteodent” was used. According to clinical and radiological methods, the using of Er:YAG laser provides less tissue trauma during surgery, the reduction or absence of pain response, decreased postoperative collateral edema, shortening of the healing time for soft tissues, formation of soft elastic scars and reparation of bone tissue. The using of the erbium laser is effective during cystectomy of the jaws.

MONITORING OF THE QUALITY OF LIFE AND PSYCHOLOGICAL HEALTH OF PATIENTS WITH DIABETES MELLITUS TYPE 2 IN THE TREATMENT OF REMOVABLE LAMINAR DENTURES

Abstract

The study of dental indicators of quality of life and the psychoemotional status of patients with type 2 diabetes with the orthopedic treatment of partial removable acrylic prostheses was carried out. In the period from 2014 to 2016 it was, carried out examination and treatment of 68 patients with partial adentia 1 and 2 classes at Kennedy in age from 51 to 68 years. The main group (n = 32) consisted of patients with diabetes type 2 diabetes with a disease duration of 6,8±1,7 years and an average blood sugar level of 8,2 and 1,2 mmol/L. Control group (n=36) consisted of patients without somatic pathology, balanced by gender and age with at group of observation. All patients in the period of orthotic treatment were defined with situational and personal anxiety according to the test Charles D. Spielberger, state of hostility and aggression questionnaire by A. Bass-A. Durka, level of neurotic test L. I. Wasserman. Quality of life related to health was assessed by questionnaire MO SF-36, determination of the degree of influence stomatologic pathology on quality of life in the “Profile of the impact of dental health” OHIP-14 RU. In the result of the study it was established positive dynamics of studied parameters of diabetic patients with orthopedic treatment of partial removable acrylic prostheses, which suggests that the reconstruction of defects of the dentition reflects not only the fact of improving dental health, but also has a significant positive impact on psychological health and quality of life of these patients.

Detailed examination with modern imaging techniques and accurate diagnosis are the key points in successful treatment and prevention of complications in this category of patients.

FREE GINGIVAL GRAFT MORPHOGENESIS

Abstract

This review analyzes the current state of the problem of creating the area of keratinized and attached gingiva in dental implantology. Apically positioned flap with a free gingival graft (FGG) placement is considered the gold standard in the oral soft tissue plastic surgery. It is noted that the periosteal or connective tissue recipient bed is an important prerequisite to graft revascularization and survival. FGGs placed on the periosteum often display clinical mobility while grafts fixed on denuded alveolar bone predictably create a firmly attached band of keratinized gingiva. The development of specific techniques and methods for fixing the FGG on bone bed have special priority. Exploring rigid fixation of the graft, as well as cellular and molecular mechanisms that determine the dynamics of the regenerative processes at the graft-bed interface has great clinical potential.

THE SPECIFICITY OF THE DENTAL STATUS IN PATIENTS WITH SQUAMOUS CELL CARCINOMA OF THE ORAL CAVITY

Abstract

In this article considered the current vision of the problem of the influence of dental status in patients on the occurrence and/or development of squamous cell carcinoma (SCC) of the mucous membrane of the mouth (MMOC). On the basis of the some studies have noted a relationship between low level of the oral hygiene and the sharp edges of the teeth or dentures and the risk of developing this disease. Also revealed a correlation between periodontal disease and the possible development of SCC MMOC that corresponds to the main prediction about the risk of the development of this pathology on the background of the effects of chronical infection in the oral cavity. Significant correlation between the presence of carious lesions and the risk of developing SCC MNOC in contrast, is not detected. But clearly expressed role in the development of the cancers are galvanism and precancerous diseases of the oral mucous membrane and the red border of the lips.
