Orthodontic neuroorthopedic pathology in children and adolescents with pain dysfunction syndrome of the temporomandibular joint

Abstract

BACKGROUND: The number of children and adolescents with temporomandibular joint pain dysfunction syndrome (TMJ SPD) is increasing annually. One of the causes of this disease is dental pathology, particularly dental anomalies.

AIM: To study the prevalence of TMJ SPD in children and adolescents and the role of orthodontic and neuroorthopedic pathology in its occurrence.

MATERIAL AND METHODS: A survey of 622 students from state educational institutions was carried out. The students were divided into two age groups according to the main period of dental formation and the age periodization scheme of human ontogenesis. The students were divided into two groups with and without orthodontic pathology, and four subgroups: subgroup 1 was children without dentofacial abnormalities (n=92); subgroup 2 was children with dentofacial anomalies (n=66); subgroup 3 was teenagers without dentofacial anomalies (n=313), and subgroup 4 was teenagers with dentofacial anomalies (n=151).

RESULTS: An increase in the prevalence of TMJ SPD was found in adolescents (weak reliable relationship, r=0.10, p <0.05). In the two age groups, correlation analysis revealed a weak reliable relationship between dental anomalies and TMJ SPD (r=0.212 for children and r=0.161 for adolescents, p <0.01); as well as a weak significant association between distal bite (r=0.131, p <0.010), and an excessively deep vertical bite with TMJ SPD (r=0.152, p <0.010) in adolescents. A significant association was detected between scoliosis and TMJ SPD (r=0.340, p <0.010) in both age groups.

CONCLUSION: An increase in the prevalence of TMJ SPD was found in adolescents. Dental anomalies that occur during adolescence may be a cause of TMJ SPD; however, the main role is played by cervical vertebral dystrophic syndrome, scoliosis, and (or) postural disorders. In all cases, TMJ SPD was diagnosed with signs of cervical vertebral dystrophic syndrome, scoliosis, and (or) postural disorders, the clinical manifestations of which depended on the age of the student. Thus, it is necessary to conduct a comprehensive multidisciplinary examination on all patients with TMJ SPD and include dentists, neurologists, pediatricians, and, if necessary, orthodontists.

Assessing the state of the alveolar morphotype when planning and carrying out orthodontic treatment for patients with inflammatory periodontal disease

Abstract

BACKGRAUND: Negative functional and anatomical disorders of the periodontal complex tissues occur in 30–55% of cases against a background of orthodontic tooth movement. New diagnostic capabilities are needed to monitor the state of periodontal tissues during orthodontic tooth movement when using removable and non-removable types of orthodontic equipment to prevent the pathology of the supporting apparatus of the tooth.

AIM: To assess the state of periodontal tissues when planning and performing controlled orthodontic movement of crowns and roots of teeth using 3D computed tomography overlay in patients with inflammatory periodontal disease.

METHODS: The study included 80 patients aged 25–35 years, who were divided into a control group (with clinically healthy periodontium) and the main (with chronic generalized periodontitis) group for orthodontic treatment of dentoalveolar anomalies. The orthodontic equipment for the groups was identical (aligners, vestibular, and lingual braces with passive self-ligation). Before starting the orthodontic program, the alveolar morphotype was determined by a patient CT superimposed on a digital modeling program of the future result; the optical density of the alveolar bone was estimated in Hounsfield units. Clinical indicators to assess the condition of the periodontal tissues and the level of tissue recession were determined before treatment, after active periodontal treatment, including resection surgery, and after completion of the orthodontic program.

RESULTS: After the completion of orthodontic treatment, patients in group 1 had fewer periodontal complications (42% in group 1 vs. 54% in group 2), with an initially normal periodontium and a thin alveolar morphotype; 18% and 23%, with a thick alveolar morphotype. The best results were noted in the aligner subgroup (group 1, 16% vs. group 2; the worst were observed in the subgroup with lingually fixed braces at 50% and 55%, respectively). The most common periodontal complication was tissue recession. A positive orthodontic result was obtained after assessing the patient's computed tomography scans superimposed on the digital modeling program of the final result after the teeth were displaced.

CONCLUSIONS: The ability to control the state of the alveolar ridge bone using 3D visualization of tooth movement with a CT overlay increased the efficiency of orthodontic treatment planning by 40%, ensuring minimal side effects and complications.

Influence of soft tissue on the reparative abilities of the jaw bone tissue in patients with dentoalveolar lesions

Abstract

BACKGROUND: Treating patients with a jaw bone defect requires eliminating the defect, restoring dentition, and providing long-term support for the functional state of the dental system. However, dental damage reduces the reparative capabilities of the jaw bone tissue. Therefore, when developing ways to repair such defects, the proportion of soft tissue in the bone defect must be determined.

AIM: To study the effect of soft-tissue elements on the reparative abilities of jaw bone tissue.

MATERIALS AND METHODS: This study included 98 people with acquired combined jaw bone defects. The material was taken during a surgical intervention to study the tissue environment and the characteristics of the transformation of the tissues surrounding the defect. The samples were sent for histological examination.

RESULTS: Microscopic examination of the histological sections obtained from the area of the jaw bone defects revealed the proliferation of a multilayer flat non-corneating epithelium with “creeping” and massive ingrowth of the epithelium into the area of the bone defect. The epithelium had advanced into the underlying bone, which led to atrophy and destruction of the bone over the entire area of the defect, increasing the volume of the defect. An epithelial-connective tissue complex lined the bone surface of the defect, replacing the periosteum.

CONCLUSIONS: The morphology of the tissues surrounding the area of a bone defect suggests a decrease in cambial bone elements. Treating jaws with bone defects requires eliminating the soft tissue that fills the bone defect, followed by guided bone regeneration using a granular osteoconductive graft and a resorbable collagen membrane.

Experimental comparison of screw loosening of full zirconia crowns supported by straight implants, straight implants with angled abutments, and angled implants with different platform inclinations: An in vitro study

Abstract

BACKGROUND: The success of placing an immediate implant in the area of the maxillary central incisor depends on the degree of tightening of the fixation screw for full zirconia crown. Clinicians use implants of various designs and angled abutments to bypass anatomical limitations, achieve a stable primary implant, and fabricate a screw-retained crown. However, a crown fabricated for an implant can experience non-axial loads during occlusion, which can loosen screws and adversely affect treatment success, particularly during the early postoperative period.

AIM: To compare the effect of cyclic loading on the reliability of the fixation screw for a full zirconia crown supported by an angled implant with platform inclinations of 12, 24, and 36 using straight implants or straight implants with an angled abutments.

MATERIALS AND METHODS: The degree of prosthetic screw tightening was studied in 30 samples (six groups of 5 implant types, such as straight implants, straight implants with standard 17° angled abutments, or 12° custom abutments and angled implants with platform inclinations of 12°, 24°, and 36°). A full zirconia crown was fabricated for the maxillary central incisor, fixed on every implant, and exposed to cyclic loading for 1×106 cycles. Then, the maximum unscrewing torque was determined for each group, and statistical analysis was carried out.

RESULTS: The minimum values of the maximum torque value was 20.804±0.01 N/cm for the group of straight implants with 17° angled abutments and the maximum value for the group of straight implants was 22.82±0.04 N/cm. However, the values in these groups were higher than those of the groups of implants with angled abutments.

CONCLUSION: This study showed that fixation screws are more reliable in crowns supported by straight implants or angled implants with different platform inclinations. Therefore, these implants are the preferred option for placing an implant in the anterior region.

Determination of the occlusal plane level and direction: A literature review

Abstract

The occlusal plane is formed at the point of contact of the occlusal surfaces of the upper and lower teeth. The position of the occlusal plane determines the location of dentition in the facial skeleton. Many patients have a change in the vertical dimension of their jaws due to tooth loss, pathological tooth abrasion, or restorations without functional anatomy, which changes the occlusal plane.

Rehabilitating such patients is associated with the difficulties of restoring the physiological height of the vertical dimension. Determining the constructive position of the mandible, which depends on the correct positioning of the height dimension and the position of the jaw in the sagittal, frontal, and transverse planes affects function and aesthetics, such as the occlusal plane.

The level, direction, and curvature of the occlusal plane are important criteria for planning and implementing restorative dental treatment.

This review presents definitions of the occlusal plane and methods for finding and constructing the occlusal plane during dental orthopedic treatment.

Multilayer dental ceramics blanks based on zirconium dioxide: A literature review

Abstract

An overview of multilayered dental ceramics based on zirconium dioxide is presented. Classifications of dental ceramics based on zirconium dioxide and types of multilayer billets made of zirconium dioxide are considered. Examples of companies producing multilayered zirconium dioxide are given, and the strength and aesthetic properties of the different layers of multilayered blanks are described. Introducing a coloring additive does not affect the mechanical strength of multilayered zirconium dioxide. An increase in the content of yttrium oxide in the composition of multilayered zirconium dioxide increases transparency but decreases mechanical strength. Multilayered ceramics with a higher degree of transparency have poorer physical and mechanical properties than less transparent materials. Sandblasting reduces the mechanical strength of multilayered zirconium dioxide. If the layers of multilayered zirconium dioxide differ in color but have the same structure, then the strength indicators of each layer of the multilayered billet are at the same level. However, if the layers of multilayered zirconium dioxide differ in structure, then their strength within one billet will differ. The advantages and disadvantages of multilayered dental ceramic blanks based on zirconium dioxide are described. The prospects of using multilayered zirconium dioxide dental ceramics in Russia are discussed.

N.M. Aleksandrov, maxillofacial surgeon, professor, and major-general of the medical service (the 100th anniversary of his birth)

Abstract

BACKGROUND: The year 2023 is the 100th anniversary of the birth of a prominent Russian maxillofacial surgeon, Doctor of Medical Sciences, Professor, and Major-General of the Medical Service, Nikita Mikhailovich Aleksandrov.

AIM: The purpose of this study was to highlight the scientific, clinical, pedagogical, and social activities of this prominent maxillofacial surgeon, as well as his services to military dentistry.

N.M. Aleksandrov was a pioneer in the development and introduction of endotracheal anesthesia in the USSR during surgical interventions of the face and neck. He is the author of several reconstructive methods, such as resection of the upper jaw with primary plastic surgery, otoplasty using Filatov stem tissues and local tissues, and lower lip plasty due to two symmetrical flaps of the upper lip. These methods were developed and implemented in clinical practice under his scientific supervision.
